Transitions

Life is filled with swift transitions. In those transitions, information and changes may come from every direction. As I reflect on my own transitions, I’m reminded about transitioning in healthcare. One of the most dangerous times for a patient is during the transition / moving from one level of care to another: from hospital to rehabilitation; skilled nursing to home. It is imperative that communication is clear. But clarity and understanding is historically difficult to achieve. If we apply the principles of health literacy, and use the evidence-based techniques, the risk of miscommunication will be lowered. The result is improved carry over, improved outcomes and improved community health.
