I spoke with a patient today who was recently discharge twice from two different hospitals. She made the statement “ they sent me home from emergency because my head was ok, but they didn’t know how sick I was from before.” She was looking for help. I’m sure this is not an unusual story. Hospital readmissions within thirty days is a measurable quality indicator that has a health literacy component. How can we as providers ensure a continuum of care for home discharges ?
